Theoretically, drugs that inhibit the CYP3A4 enzymes might increase levels of lovastatin from red yeast rice.
Red yeast rice contains varying levels of the statin drug lovastatin, which is metabolized by CYP3A4. Combining red yeast rice with CYP3A4 inhibitors might increase serum levels of lovastatin from red yeast rice.

Here's the thing to know about this pair: it's really the red yeast rice that's the active player, not the octreotide. Red yeast rice contains lovastatin, a natural statin that your liver clears using an enzyme called CYP3A4. Octreotide can mildly slow that same enzyme down, so in theory the lovastatin could build up a bit and you'd get a stronger statin effect than expected.

If that happened, the things to watch for are muscle aches, weakness, or unusual tenderness, and rarely some liver irritation. I'll be straight with you though, this concern is theoretical, based on how the drugs work on paper rather than real reports of problems. Still, since the dose of lovastatin in red yeast rice varies a lot, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Octreotide Used For?

Brand names: Sandostatin, Mycapssa

Octreotide injection (available as Sandostatin, BYNFEZIA Pen, and other octreotide acetate injection products) is used to lower abnormally high growth hormone and IGF-1 levels in acromegaly, and to reduce the severe diarrhea and flushing associated with metastatic carcinoid tumors and the profuse watery diarrhea caused by VIPomas (vasoactive intestinal peptide-secreting tumors).

Mycapssa, the oral delayed-release capsule, is approved specifically for long-term maintenance treatment of acromegaly in patients who have already responded to and tolerated treatment with octreotide or lanreotide injections. It is not indicated for carcinoid tumors or VIPomas.
