Here is the interaction we found between these two, with the evidence behind it.
HEPATOTOXIC DRUGS
Why this applies to Tamoxifen Citrate
Tamoxifen Citrate falls under HEPATOTOXIC DRUGS — the group Red Yeast Rice interacts with.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern when you pair red yeast rice with tamoxifen: both can put a little extra stress on your liver. Red yeast rice naturally contains lovastatin, a statin that occasionally raises liver enzymes or, rarely, triggers liver inflammation. Tamoxifen, on its own, can also affect the liver in some people. Stacking the two together could, in theory, add up.
I want to be honest about the strength of this, though. It's a theoretical caution based on how these substances behave, not proof that problems will happen. This isn't about red yeast rice changing how well tamoxifen fights breast cancer. Still, since tamoxifen is an important medicine, please check with your pharmacist or doctor before adding red yeast rice. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness.

About Red Yeast Rice
What is Red Yeast Rice?
- High cholesterol
- Heart health
- Metabolic syndrome
- Statin alternative
Red yeast rice contains monacolin K, which is chemically identical to the prescription statin lovastatin, and it can lower LDL ('bad') cholesterol. Because of this, it carries the same risks as statin drugs, and the amount of active ingredient varies widely between products. Always talk to your pharmacist or doctor before using it, especially if you have liver problems or take other medicines.
About Tamoxifen
What Is Tamoxifen Used For?
Brand names: Nolvadex, Soltamox
Tamoxifen is used to treat metastatic breast cancer (breast cancer that has spread) in both women and men, and as adjuvant (after-surgery) therapy for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.
In women with ductal carcinoma in situ (DCIS) — a very early, non-invasive form of breast cancer — tamoxifen is used after breast surgery and radiation to reduce the risk of developing invasive breast cancer. It is also approved to reduce the incidence of breast cancer in women who are at high risk but have not yet been diagnosed. Soltamox (oral solution) is specifically indicated for all of these uses in adult patients.
