Rhodiola and Losartan Potassium

These two interact in 4 distinct ways. Each one is explained separately below.

Why Are There 4 Interactions?

Rhodiola interacts with whole categories of medication. Losartan Potassium falls into 4 of those categories, so it can interact with Rhodiola in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ANTIHYPERTENSIVE DRUGS

1 of 4

Why this applies to Losartan Potassium

Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Rhodiola interacts with.

Theoretically, taking rhodiola with antihypertensive drugs might increase the risk of hypotension.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the simple version: losartan (Cozaar) already lowers your blood pressure, and lab and animal studies hint that Rhodiola may nudge blood pressure down a little too. Stack those two together and, in theory, your pressure could dip lower than intended. That might show up as feeling lightheaded, dizzy, or woozy, especially when you stand up quickly, or just feeling unusually tired.

I want to be honest about the evidence though. This concern comes only from test-tube and animal research, not from people, so it's more of a 'keep an eye out' than a sure thing. Losartan isn't a prodrug here, so the direction is straightforward: both can lower pressure. If you'd like to try Rhodiola, check with your pharmacist or doctor first, and watch how you feel.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 4

Losartan Potassium also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.

Here's something worth knowing if you take losartan (Cozaar) and are thinking about Rhodiola. Losartan is actually a bit unusual: it's a prodrug, meaning your liver has to convert it (using an enzyme called CYP2C9) into the active form that does most of the blood pressure lowering work. Rhodiola appears to slow that same enzyme down a little.

So in theory, Rhodiola could leave you with slightly less of the active form, which might make losartan a touch less effective at controlling your blood pressure. The good news is the effect seen in studies was small (about a 21% change), and the evidence is limited and varies from product to product. I wouldn't panic, but I'd run it by your pharmacist or doctor first, and keep an eye on your home blood pressure readings if you do try it.

LOSARTAN (Cozaar)

3 of 4
Rhodiola might increase the levels and adverse effects of losartan.
A clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for losartan (Cozaar): your liver uses an enzyme called CYP2C9 to turn losartan into its active form. Rhodiola appears to slow that enzyme down a bit. In one small study of healthy young men, taking rhodiola for two weeks cut losartan's processing by about 21%. So in theory you'd have slightly more of the parent drug hanging around and a touch less of the active piece.

In real life this is a modest effect, and the evidence is limited. Still, if you notice things like lightheadedness, dizziness when standing, or unusually low blood pressure, that's worth mentioning. Please don't start or stop anything on your own. Just give your pharmacist or doctor a heads-up before adding rhodiola so they can keep an eye on your blood pressure.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

4 of 4

Losartan Potassium also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Severity Minor Likelihood Possible Evidence B

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something worth knowing about losartan: it's actually a prodrug. Your liver uses an enzyme called CYP3A4 (along with CYP2C9) to convert losartan into its more active form, which does most of the blood-pressure-lowering work. So this one works a little differently than you might expect.

In theory, rhodiola could slow that CYP3A4 enzyme down. If it did, you might convert less losartan into its active form, which could mean slightly weaker blood pressure control. That said, the evidence here is thin. This is mostly lab-dish data, the effect varies a lot by product, and a human study found rhodiola didn't actually change how a similar CYP3A4 drug was processed. So real-world impact is likely small. If you're starting rhodiola, just keep an eye on your blood pressure readings and check with your pharmacist.

  • Stress and coping (adaptogen)
  • Mental performance and focus
  • Mild depression
  • Physical and exercise performance

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
