Rhodiola and Perindopril Arginine, Amlodipine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Rhodiola interacts with whole categories of medication. Perindopril Arginine, Amlodipine falls into 2 of those categories, so it can interact with Rhodiola in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Perindopril Arginine, Amlodipine

Perindopril Arginine, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Rhodiola interacts with.

Theoretically, taking rhodiola with antihypertensive drugs might increase the risk of hypotension.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Prestalia is a two-in-one blood pressure pill. Perindopril is an ACE inhibitor (it relaxes blood vessels by blocking the angiotensin-converting enzyme), and amlodipine is a calcium channel blocker that also widens vessels. Here's the wrinkle: lab and animal studies hint that Rhodiola may block that same ACE enzyme and gently nudge blood pressure down on its own.

So in theory, pairing them could stack their effects and push your pressure a bit too low. That might show up as dizziness, lightheadedness, feeling faint when you stand up, or unusual tiredness. I'll be honest though, this is only based on test-tube and animal data, so it's a soft signal, not a proven problem. If you'd like to try Rhodiola, check with your pharmacist or doctor first and keep an eye on your numbers.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Perindopril Arginine, Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo pill pairs amlodipine (a calcium channel blocker) with perindopril (an ACE inhibitor that the body converts into its active form). Of the two, amlodipine is the piece that's cleared by the liver enzyme CYP3A4. In a test tube, rhodiola can slow that enzyme down a bit, which in theory could let amlodipine build up and nudge your blood pressure lower than expected, or bring on more dizziness, ankle swelling, flushing, or a headache.

That said, the real-world evidence is reassuring. When researchers gave people rhodiola for two weeks, it didn't actually slow CYP3A4 in the body. So this is mostly a theoretical, minor concern. Still, it's worth a quick mention to your pharmacist or doctor, especially if you notice new lightheadedness.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress.
