Aspirin, Butalbital, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rooibos interacts with.
In vitro research shows that the methanol extract of rooibos leaves and stems inhibits CYP1A2 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters in your Fiorinal with Codeine: the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and lab studies (test tube only, not people) hint that rooibos may slow that enzyme down a bit. If that happens, caffeine could hang around longer and build up, so in theory you might feel more jittery, have a racing heart, or trouble sleeping.
The good news is this is theoretical and based on very early research, so a cup or two of rooibos tea is unlikely to cause real trouble for most folks. The aspirin, butalbital, and codeine parts aren't really affected by this. If you use concentrated rooibos extracts or notice extra restlessness, check in with your pharmacist or doctor.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo. The codeine in Fiorinal with Codeine is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Lab research hints that rooibos might slow down that same enzyme. If that happened, less codeine would convert to morphine, so you could get less pain relief rather than more.
The good news: this is only test-tube data, so we don't really know if drinking rooibos tea affects people this way. A cup or two of tea is unlikely to cause trouble. Concentrated rooibos extracts are more of a question mark. If you rely on this medicine for pain, just mention your rooibos use to your pharmacist or doctor.

In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Fiorinal with Codeine. A couple of the ingredients in this combo, especially the butalbital (a barbiturate) and codeine, are partly handled by a liver enzyme called CYP3A4. Lab studies hint that concentrated rooibos extract may slow that enzyme down a bit. If that happened in real life, these ingredients could linger longer and feel stronger, meaning more drowsiness, dizziness, slowed breathing, or grogginess.
That said, this is only a test-tube finding, not something proven in people. Drinking rooibos as a normal cup of tea is very unlikely to cause trouble. The bigger question would be concentrated rooibos supplements. Since this medication already carries sedation risks, it's worth a quick chat with your pharmacist or doctor before adding any rooibos extract.
