Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down and clear bedaquiline. Lab studies hint that concentrated rooibos extract may slow that enzyme down. If that happened in real life, bedaquiline could build up a bit higher than expected, which in theory could nudge up side effects. With this drug, the one to keep an eye on is the heart rhythm (it can prolong the QT interval), along with nausea, headache, or liver enzyme changes.

I want to be honest about the strength here: this is only test-tube evidence, not human data, so it may not matter much in practice. A regular cup or two of rooibos tea is very different from a concentrated extract. Since bedaquiline is part of a carefully managed TB regimen, just mention any rooibos supplements to your TB team or pharmacist before adding them.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
