Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

Here's the gist for irinotecan (Onivyde): one of the enzymes your body uses to help process this chemo drug is called CYP3A4. Lab tests show that a concentrated rooibos extract can block that enzyme. If that happened in people, irinotecan could clear more slowly and build up, which might make its side effects stronger. With this drug that can mean worse diarrhea, low blood counts, nausea, and fatigue, so it's worth taking seriously.

That said, this is only a theoretical concern based on a test-tube study, not human data, and it used a strong extract rather than a cup of tea. Still, since irinotecan is potent chemo, please run any rooibos tea or supplements by your oncology team or pharmacist first.
