Tapentadol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP2C19 substrates may increase the effects of CYP2C19 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP2C19 enzyme activity.

Here's the gist for tapentadol (Nucynta). The concern on file is that rooibos might block a liver enzyme called CYP2C19, which can slow how some medicines are broken down. Tapentadol is an active opioid on its own (it is not a prodrug that needs activating), so if its breakdown slowed, more drug could linger in your body. In theory that could mean stronger opioid effects, like extra drowsiness, dizziness, constipation, nausea, or slowed breathing.

That said, this comes only from lab and test-tube research, not real people, and CYP2C19 is only a minor pathway for tapentadol. A cup or two of rooibos tea is very unlikely to cause trouble. Concentrated extracts are a bigger unknown, so check with your pharmacist or doctor before using those.

Here's the gist with rooibos and tapentadol. Some of tapentadol's breakdown runs through a liver enzyme called CYP2C9, and very early lab work suggests rooibos extract might slow that enzyme down a touch. In theory, that could let a little more tapentadol linger in your system, which might mean stronger opioid effects like extra drowsiness, dizziness, nausea, constipation, or slower breathing.

That said, I want to be honest about how thin this is. The evidence comes only from test-tube studies, not real people, so a regular cup or two of rooibos tea is unlikely to cause trouble. Concentrated rooibos extracts are the bigger question mark. Since tapentadol is a serious opioid, please run any rooibos supplements past your pharmacist or doctor first.

About Tapentadol

What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
