Vandetanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rooibos interacts with.

Theoretically, taking rooibos with CYP3A4 substrates may increase the effects of CYP3A4 substrates.
In vitro research shows that the methanol extract of rooibos leaves and stems strongly inhibits CYP3A4 enzyme activity.

Here's the gist for vandetanib (Caprelsa). Your body partly clears this cancer drug using a liver enzyme called CYP3A4. Lab tests (in test tubes, not people) hint that concentrated rooibos extract may slow that enzyme down. If that happened in the body, vandetanib could build up a bit, which in theory could mean more of its side effects like diarrhea, rash, high blood pressure, or changes to your heart rhythm (QT prolongation).

I want to be honest: this is theoretical and based only on early lab work, not human studies. A normal cup of caffeine-free rooibos tea is very unlikely to cause trouble. Concentrated extracts are the bigger question mark. Since vandetanib is a serious medication, please run any rooibos supplements past your oncologist or pharmacist first.

Caprelsa (vandetanib) is used to treat medullary thyroid cancer — a type of thyroid cancer — in adults whose tumor cannot be surgically removed or has spread (locally advanced or metastatic disease) and is causing symptoms or actively growing.

Caprelsa should be used with extra caution in patients whose cancer is slow-growing and not causing symptoms, because the serious risks of the medicine must be carefully weighed against any potential benefit in that setting.
