Trandolapril, Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 1A2. Theoretically, schizonepeta might increase the effects and side effects of CYP1A2 substrates.
Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline, zileuton (Zyflo), zolmitriptan (Zomig), and others.

Tarka combines trandolapril (a blood pressure medicine) with verapamil (a calcium channel blocker that also slows the heart). The concern on file is that schizonepeta may block a liver enzyme called CYP1A2. In theory, slowing that enzyme could let a drug it clears build up a bit, leading to stronger effects.

Here's the honest part: this is based only on lab and animal studies, so it's very theoretical. Verapamil is mostly handled by a different enzyme (CYP3A4), not CYP1A2, so a big effect is unlikely. Still, if levels did rise, you might notice low blood pressure, dizziness, a slow heartbeat, or swelling. Occasional culinary use is far less concerning than concentrated supplements. Please check with your pharmacist or doctor first.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Tarka pairs trandolapril with verapamil, and verapamil is the piece that matters here. Your liver uses an enzyme called CYP3A4 to clear verapamil. Some animal lab work hints that a compound in Schizonepeta may speed that enzyme up. If that happened in people, your body could clear verapamil a little faster, which might soften its effect. In real terms, that could mean slightly less blood pressure control or a touch less of its heart-rate-slowing benefit.

Honestly, this is theoretical and rests only on early animal and test-tube data, so I wouldn't lose sleep over it. Still, since blood pressure meds are worth keeping steady, check with your pharmacist or doctor before adding Schizonepeta, and watch your home readings if you do.
