Hydrochlorothiazide, Labetalol falls under HEPATOTOXIC DRUGS — the group Spearmint interacts with.

Theoretically, high doses of spearmint might increase the risk of liver damage when taken with hepatotoxic drugs.
Animal research suggests that drinking spearmint tea for 30 days can increase markers of liver damage, including aspartate aminotransferase (AST) and alanine aminotransferase (ALT), and cause liver degeneration and necrosis, in a dose-dependent manner. This effect has not been reported in humans.

Here's the honest picture with your blood pressure combo (hydrochlorothiazide plus labetalol) and spearmint. The concern on file is about the liver, not about spearmint changing how your medicine works. Labetalol in particular is processed by the liver and, rarely, can stress it. The thinking is that very high, concentrated doses of spearmint might add to that strain.

But I want to be clear: this is theoretical. It comes from animal studies where rats drank strong spearmint tea for a month, not from real cases in people. Normal food and tea amounts of spearmint are not a worry. If you're considering a concentrated spearmint supplement, just run it by your pharmacist or doctor first, especially if you've had any liver issues.
