Hydrochlorothiazide, Methyldopa falls under HEPATOTOXIC DRUGS — the group Spearmint interacts with.
Animal research suggests that drinking spearmint tea for 30 days can increase markers of liver damage, including aspartate aminotransferase (AST) and alanine aminotransferase (ALT), and cause liver degeneration and necrosis, in a dose-dependent manner. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your blood pressure combo of hydrochlorothiazide and methyldopa. Methyldopa is one of the few blood pressure medicines that can, in rare cases, irritate the liver on its own. The concern on file is purely theoretical: very high, concentrated doses of spearmint might add a little extra stress to the liver when paired with a drug that can already affect it.
This comes only from animal studies where large amounts of spearmint raised liver enzymes (AST and ALT). It has not been seen in people, and ordinary spearmint in tea or food is not expected to be a problem. If you take strong spearmint supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on your liver tests.
