Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the gist for Cabenuva, which combines cabotegravir and rilpivirine. The rilpivirine part is partly cleared by a liver enzyme called CYP3A4. In test-tube and animal studies, sulforaphane appears to slow that enzyme down. In theory, that could let rilpivirine levels climb a bit, which might mean more of its usual effects, things like headache, trouble sleeping, mood changes, or fatigue.

I want to be honest about the strength here: this is theoretical only and has never been reported in people. Everyday broccoli or other cruciferous veggies in your diet are very unlikely to matter. A concentrated sulforaphane supplement is the real question. Since keeping HIV treatment steady is important, please check with your pharmacist or doctor before adding one.

What Is Cabotegravir and Rilpivirine Injections Used For?

Brand name: Cabenuva

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
