Swamp milkweed contains cardioactive steroids known as cardenolides that may have digitalis-like activity. Potassium depletion can increase the risk of cardenolide toxicity.

Here's the concern with this combo. Your medication contains hydrochlorothiazide, a water pill that can lower your body's potassium over time. Swamp Milkweed contains heart-active compounds (cardenolides) that act a bit like the old heart drug digoxin. When potassium runs low, the heart becomes more sensitive to these compounds, which can raise the risk of irregular or dangerous heart rhythms.

I want to be honest about the evidence: this is theoretical, based on how these substances behave, not on real studies in people. That said, Swamp Milkweed isn't a proven supplement and its sap is genuinely toxic, so I wouldn't recommend taking it at all. If you've been using it or are considering it, please talk with your pharmacist or doctor first.
