Amlodipine Besilate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.

Here's the gist with amlodipine (Istin): your body relies on a liver enzyme called CYP3A4 to break it down and clear it out. Lab studies suggest Terminalia arjuna may slow that enzyme. If that happens, more amlodipine could linger in your system, which means its blood-pressure-lowering effect (and side effects) could be a bit stronger than usual. With amlodipine, that might show up as swollen ankles, lightheadedness, flushing, or a faster heartbeat.

I'll be straight with you though: this is theoretical, based only on test-tube research, not real patients. So it isn't cause for alarm. Still, since amlodipine affects your blood pressure, it's worth a quick chat with your pharmacist or doctor before combining the two, especially if you already have a heart condition.
