Terminalia and Aspirin, Butalbital, Caffeine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Terminalia interacts with whole categories of medication. Aspirin, Butalbital, Caffeine falls into 2 of those categories, so it can interact with Terminalia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Aspirin, Butalbital, Caffeine

Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP3A4 enzymes and reduces CYP3A4 substrate metabolism.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist with Fiorinal, which combines aspirin, butalbital, and caffeine. Lab studies hint that Terminalia arjuna may slow down a liver enzyme called CYP3A4. The butalbital piece (a barbiturate) leans on that enzyme to be cleared from your body, so in theory it could linger a bit longer and add up. That might mean extra drowsiness, dizziness, or feeling foggy or sluggish.

I want to be straight with you though: this is only test-tube data, not real people, so it's more of a 'keep an eye out' than a red alert. Everyday spice-level amounts are unlikely to matter, but concentrated supplements are a different story. Before pairing them, just run it by your pharmacist or doctor.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Aspirin, Butalbital, Caffeine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the piece to keep an eye on with Fiorinal: it contains aspirin, which already thins your blood a bit by making your platelets less sticky. Terminalia arjuna appears to do something similar, so stacking the two could, in theory, add up and make bleeding a little more likely. Think easy bruising, nosebleeds that take longer to stop, bleeding gums, or stomach upset and dark stools from the aspirin side of things.

I want to be honest about the strength of this, though. The concern comes from lab and test-tube studies, not from people, so it's more of a caution flag than a proven problem. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up before combining them, especially if you bleed easily or take other blood thinners.

From our pharmacist’s overview on the Aspirin, Butalbital, Caffeine page

“The aspirin inside Fiorinal (aspirin, butalbital, caffeine) is the reason this list deserves careful reading rather than a quick skim. More than 200 of the supplements and herbs on file can thin the blood further and raise bleeding risk on top of aspirin's own antiplatelet effect, and even ordinary alcohol, beer and wine included, carr…”
