Azilsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.

Here's the gist: your body uses an enzyme called CYP2C9 to help clear azilsartan (Edarbi). Lab studies suggest that Terminalia arjuna may slow that enzyme down a bit. If that happened in real life, azilsartan could hang around longer and have a slightly stronger effect than usual. Since azilsartan already lowers blood pressure, the main thing to watch for would be your pressure dropping too low, leaving you feeling dizzy, lightheaded, or tired, especially when you stand up quickly.

I don't want to overstate this. This warning comes only from test-tube research, not from studies in people, so it's theoretical at this point. Both Terminalia and azilsartan can also nudge potassium levels up, which is another reason to loop in your pharmacist or doctor before combining them.

What Is Azilsartan Used For?

Brand name: Edarbi

Azilsartan medoxomil tablets — sold as both the generic Azilsartan Medoxomil and the brand Edarbi — are approved to treat high blood pressure (hypertension) in adults. Bringing blood pressure down lowers the risk of serious cardiovascular events, especially stroke and heart attack.

The medication can be used on its own or alongside other blood pressure medicines. It is not a cure for high blood pressure, and managing the condition fully often involves lifestyle steps like limiting sodium, exercising, and quitting smoking.
