Insulin Degludec, Liraglutide falls under ANTIDIABETES DRUGS — the group Terminalia interacts with.

Theoretically, concomitant use of Terminalia bellirica or Terminalia chebula with antidiabetes drugs could affect blood sugar control and increase the risk of hypoglycemia.
Animal and in vitro research shows that Terminalia bellirica and Terminalia chebula fruit and seed extract have hypoglycemic effects.

Xultophy already lowers your blood sugar in two ways: the insulin degludec part and the liraglutide part. Here's the thing with Terminalia. In lab dishes and animal studies, certain species (Terminalia bellirica and chebula) seemed to lower blood sugar on their own. If that holds true in people, stacking it on top of Xultophy could push your sugar down a little too far.

I want to be honest about how soft this evidence is. This is a theoretical concern based on pharmacology, not real human data, so I wouldn't lose sleep over it. Still, watch for shakiness, sweating, dizziness, or confusion, the usual low blood sugar signs. Loop in your pharmacist or doctor before adding it.
