Irbesartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.

Here's the gist: your body uses a liver enzyme called CYP2C9 to help break down irbesartan. Lab studies suggest that Terminalia arjuna may slow that enzyme down. If that happened in real life, irbesartan could clear more slowly and build up a bit, nudging its blood-pressure-lowering effect stronger than expected. That might show up as dizziness, lightheadedness (especially standing up), tiredness, or a blood pressure that runs lower than usual.

I want to be honest about the strength of this, though. It's only seen in test-tube research, not in actual people, so it's more of a theoretical concern than a proven problem. Still, since both lower blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them, and keep an eye on how you feel.

Terminalia is a group of traditional Ayurvedic tree species (most notably Terminalia arjuna) used for heart, digestive, and general wellness purposes. What Is Irbesartan Used For?

Brand names: Avapro, Aprovel

Irbesartan tablets — sold as Avapro and under the generic name Irbesartan — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events, including stroke and heart attack. The medication can be used alone or combined with other blood pressure medicines.

Irbesartan tablets are also used to treat diabetic nephropathy (kidney disease caused by diabetes) in adults who have type 2 diabetes, high blood pressure, elevated creatinine levels, and significant protein in the urine. In these patients, it slows the worsening of kidney disease.
