Why this applies to Piroxicam

Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Terminalia interacts with.

Theoretically, use of Terminalia arjuna may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Terminalia arjuna extract inhibits CYP2C9 enzymes and reduces CYP2C9 substrate metabolism.

Here's the gist for Feldene (piroxicam): your liver relies on an enzyme called CYP2C9 to break this drug down and clear it. Lab studies suggest that Terminalia arjuna may slow that same enzyme. If it does, piroxicam could hang around longer and build up, which would mean stronger effects and a higher chance of the usual NSAID side effects, things like stomach upset, heartburn, ulcers or bleeding, and fluid retention or higher blood pressure.

That said, this is based only on test-tube data, so it's a theoretical concern, not a proven problem in people. Piroxicam is not a prodrug, so the direction here is a possible increase in its effect. I wouldn't panic, but please run it by your pharmacist or doctor before combining them, especially if you have heart issues.

Theoretically, concomitant use of Terminalia arjuna with anticoagulant or antiplatelet drugs may increase the risk of bleeding in some patients.
In vitro, Terminalia arjuna bark extract inhibits platelet aggregation, decreases platelet activation, and shows antithrombotic properties.

Here's the thing to keep in mind with piroxicam: it's an NSAID, and NSAIDs already nudge up your bleeding risk a bit by making platelets (the cells that help your blood clot) less sticky. Piroxicam can also be tough on the stomach lining, which raises the chance of stomach irritation or bleeding.

Now, lab studies suggest Terminalia arjuna may also make platelets less likely to clump together. So in theory, pairing the two could add up and bump up your bleeding risk a little, things like easy bruising, nosebleeds, or stomach upset. I want to be honest, though: this is based only on test-tube and animal research, not real patients, so it's more of a caution than a proven problem. Just give your pharmacist or doctor a heads-up before combining them, especially if you bruise easily or have any stomach issues.
