Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the gist with this pairing: it's the thyroid extract, not your sulfasalazine, that may get shortchanged. When you swallow these two close together, sulfasalazine sitting in your stomach and gut can bind up some of the thyroid hormone and keep your body from absorbing all of it. Over time that could mean your thyroid replacement works a little less well, so symptoms like fatigue, feeling cold, or sluggishness might creep back.

The good news is this is a minor, timing-based issue, and it's easy to manage by spacing the two doses apart. Thyroid hormone is best on an empty stomach, well before other pills. Your sulfasalazine effect for colitis or arthritis shouldn't change here. Since OTC thyroid extract is unpredictable to begin with, please loop in your pharmacist or doctor before mixing these.

What Is Sulfasalazine Used For?

Brand names: SAS, Azulfidine EN-Tabs, Azulfidine

Sulfasalazine tablets and delayed-release tablets are both used to treat mild to moderate ulcerative colitis, as add-on therapy in severe ulcerative colitis, and to help keep ulcerative colitis in remission between flares.

The delayed-release form — sold as Azulfidine EN-tabs and Sulfasalazine Delayed Release Tablets — is also approved for adults with rheumatoid arthritis who have not responded well enough to NSAIDs or salicylates, and for children with polyarticular-course juvenile rheumatoid arthritis who have had a similar inadequate response.
