Turkey Tail Mushroom and rosiglitazone, glimepiride

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Turkey Tail Mushroom interacts with whole categories of medication. rosiglitazone, glimepiride falls into 2 of those categories, so it can interact with Turkey Tail Mushroom in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIDIABETES DRUGS

1 of 2

Why this applies to rosiglitazone, glimepiride

rosiglitazone, glimepiride falls under ANTIDIABETES DRUGS — the group Turkey Tail Mushroom interacts with.

Theoretically, taking turkey tail mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research suggests that turkey tail mushroom and a polysaccharide component can have hypoglycemic effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Avandaryl combines two diabetes medicines: glimepiride, which pushes your body to release more insulin, and rosiglitazone, which makes your cells more sensitive to insulin. Both already work to lower blood sugar. The concern here is that turkey tail mushroom may have its own mild blood-sugar-lowering effect, so stacking it on top could, in theory, push your numbers a little lower than expected. With glimepiride in the mix, that matters most, since that part of the combo can already cause real hypoglycemia (shakiness, sweating, dizziness, confusion).

Honestly, this is a theoretical worry based only on animal studies, not strong human evidence, so I wouldn't panic. Just keep an eye on your readings and watch for low-sugar symptoms. Please loop in your pharmacist or doctor before adding turkey tail, and don't change your Avandaryl dose on your own.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 2

rosiglitazone, glimepiride also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, the polysaccharide peptide (PSP) component of turkey tail mushroom might inhibit CYP2C9.
Laboratory research suggests that the PSP component of turkey tail mushroom dose-dependently inhibits CYP2C9. Theoretically, taking PSP with drugs metabolized by CYP2C9 might increase drug levels and the risk of adverse effects. However, this has not been reported in humans.

Avandaryl is a combination of two diabetes medicines, rosiglitazone and glimepiride, and both rely partly on a liver enzyme called CYP2C9 to be cleared from your body. Turkey tail mushroom has a component called PSP that, in laboratory tests, slowed down this same enzyme. In theory, that could let these medicines build up a bit higher than usual.

If that happened, the main concern would be glimepiride pushing your blood sugar too low. Watch for shakiness, sweating, dizziness, confusion, or a racing heart. I want to be honest though: this is only seen in lab dishes, not in real people, and it is rated a minor, theoretical interaction. Still, since you are managing diabetes, mention the mushroom to your pharmacist or doctor and keep an eye on your blood sugar readings.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the rosiglitazone, glimepiride page

“Siberian cocklebur worries me most on this page. Its seeds have caused severe, worsening drops in blood sugar in people, and rosiglitazone with glimepiride (Avandaryl) already pulls glucose down through two separate mechanisms, so these pairings deserve real vigilance.”

About Turkey Tail Mushroom

What is Turkey Tail Mushroom?

Turkey Tail Mushroom
  • Immune support
  • Used alongside cancer care
  • Gut and digestive health
  • General wellness and energy
  • Antioxidant support

Turkey tail is a woodland mushroom rich in polysaccharides like beta-glucans, and it is mainly used to support the immune system. Some of its purified extracts (PSK and PSP) have been studied as add-on treatments alongside conventional cancer therapy in Asia, but evidence for everyday supplement use is limited. It is generally well tolerated, but talk to your pharmacist or doctor before using it, especially if you have a health condition or take other medicines.
