Here's the gist with turmeric and Selzentry (maraviroc). This isn't about turmeric changing how your HIV medicine works. Instead, the worry is on the liver side. Maraviroc can occasionally stress the liver, and there have been scattered reports of concentrated turmeric or curcumin products causing liver trouble too. Stack two things that can each be hard on the liver, and the theoretical concern is a slightly higher chance of liver irritation.

I want to be honest that this is based on weak, anecdotal evidence, not solid proof. The spice in your food is very unlikely to matter. The bigger question is high-dose, highly absorbable curcumin supplements. If you ever notice yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, call your doctor. Before starting a turmeric supplement, check with your pharmacist or HIV provider.

Maraviroc also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the deal with taking turmeric alongside Maraviroc (Selzentry). Your body relies on a liver enzyme called CYP3A4 to break down maraviroc and clear it out. Lab and animal studies suggest concentrated turmeric, mainly its curcumin, may slow that enzyme down. If that happens in people, maraviroc could build up a bit and you might feel more of its side effects, like dizziness, headache, stomach upset, or feeling unusually tired.

I want to be honest with you: this is mostly theoretical, based on test-tube and animal research, not solid human studies. The amount you'd sprinkle on food is very unlikely to cause trouble. Concentrated supplement doses are the real question. Since maraviroc is part of your HIV treatment, please loop in your pharmacist or doctor before adding a turmeric supplement.

Some research is promising, but quality is mixed and curcumin is poorly absorbed on its own. About Maraviroc

What Is Maraviroc Used For?

Brand names: Celsentri, Selzentry

Maraviroc treats a specific type of HIV-1 infection called CCR5-tropic HIV-1. It is always used alongside other antiretroviral (anti-HIV) medicines — never alone. A special test called a tropism assay must be done before starting treatment to confirm you have the right type of HIV.

Selzentry (the brand-name oral solution and tablet) is approved for adults and pediatric patients weighing at least 2 kg. Generic maraviroc tablets are approved for adults and pediatric patients aged 2 years and older weighing at least 10 kg. Neither form is recommended for patients whose HIV is classified as dual/mixed-tropic or CXCR4-tropic.
