Why this applies to Tepotinib

Tepotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear tepotinib out of your system. Turmeric, mainly its active compound curcumin, can nudge that same enzyme. Most of the signal we have points to turmeric blocking CYP3A4, which would slow the breakdown of tepotinib and let more of it pile up. With a cancer drug like this, higher levels can mean more nausea, swelling (fluid retention), tiredness, and changes in liver or kidney lab values.

Now, the honest part: this is based mostly on lab and animal studies, plus a handful of case reports with other cancer pills, not tepotinib itself, so it's a possibility rather than a sure thing. A sprinkle of turmeric in your cooking is unlikely to cause trouble. Concentrated capsules are a different story. Please check with your oncology team or pharmacist before adding a turmeric supplement.

Theoretically, turmeric might increase the absorption of P-glycoprotein substrates.
In vitro and animal research shows that curcuminoids and other constituents found in turmeric can inhibit P-glycoprotein expression and activity.

Here's the gist with tepotinib (Tepmetko) and turmeric. Your body uses a kind of pump called P-glycoprotein to move certain drugs around and push some of them back out of your gut. Lab and animal studies suggest the curcumin in turmeric can slow that pump down. In theory, that could let a little more tepotinib get absorbed, which might mean stronger effects and a bump in its usual side effects like nausea, swelling in the legs or feet, diarrhea, or tiredness.

I want to be honest though: this is theoretical, based only on test-tube and animal data, and it's rated as a minor concern. Everyday turmeric in food almost certainly won't matter. Concentrated curcumin supplements are the bigger question, so since tepotinib is a serious cancer drug, just run any supplement past your oncologist or pharmacist first.

Tepmetko (tepotinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — lung cancer that has spread — whose tumors carry a specific genetic alteration called a MET exon 14 skipping mutation. A test on tumor tissue or blood is required to confirm this mutation before treatment begins.

This is a targeted therapy, meaning it works against a specific molecular driver of cancer growth rather than attacking all rapidly dividing cells.
