Folate (vitamin B9) works alongside B12 in cell division and DNA synthesis. S-Adenosyl-L-Methionine, or SAMe, is a compound your body makes naturally and uses in mood regulation, joint health, and liver function.

The tablet also contains several inactive ingredients—microcrystalline cellulose, methacrylic acid copolymer, titanium dioxide, silica, triethyl citrate, magnesium stearate, stearic acid, mannitol, and carboxymethylstarch sodium salt—which serve as fillers, binders, and flow agents to hold the tablet together.

Vitamin B12 is generally very well tolerated — your body excretes excess amounts in urine, and there's no established upper limit. It's considered safe during pregnancy and breastfeeding at recommended amounts.

Rare serious side effects from B12 include severe low potassium (hypokalemia) when correcting certain blood disorders, and allergic reactions such as rash or itching are uncommon but possible, especially in people with a cobalt allergy. SAMe is often well tolerated short-term but can cause stomach upset—nausea, diarrhea, constipation, heartburn, and loss of appetite are the most common side effects, especially at higher doses.

One caution: SAMe may trigger mania in people with bipolar disorder. Safety in pregnancy and breastfeeding is not well established, so talk with your provider.

Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
