Orally, stinging nettle is generally well tolerated when used as directed, though quality and evidence vary by product. The most common side effects are constipation and diarrhea.

Rare case reports describe liver injury after taking herbal products containing stinging nettle mixed with other ingredients — watch for abdominal pain or signs of liver trouble (yellowing skin or eyes, dark urine) and stop and seek care if they occur. Stinging nettle is not recommended during pregnancy because there isn't enough safety data and there are traditional concerns about effects on the uterus.

Safety during breastfeeding hasn't been established either, so talk with your doctor or pharmacist before use if you're nursing. If you touch fresh stinging nettle leaves or stems, expect localized itching, rash, and stinging; direct contact with the fresh plant (like placing a leaf on your tongue) can cause severe swelling and pain.
