Total Mitochondria contains three active ingredients. MicroActive Resveratrol is a compound from grapes and red wine that may support heart health and metabolism.
MicroActive CoQ10 is a nutrient naturally made by your body that fuels cellular energy production, particularly in heart and muscle cells. MicroPQQ is a third ingredient we cannot evaluate because we hold no safety or interaction data for it.
The product also includes several inactive ingredients — a vegetarian capsule, potato starch, rice hull concentrate, and other binders and anti-caking agents.
Does it work?
Strong evidence
Resveratrol is rated possibly effective for obesity and hay fever (allergic rhinitis), but possibly ineffective for heart disease and high cholesterol according to our data. CoQ10 is rated likely effective for coenzyme Q10 deficiency (when your body isn't making enough on its own), and possibly effective for fibromyalgia, migraine headache, heart failure, and diabetic nerve pain.

How safe is it?
Well-documented data
Resveratrol is generally well tolerated at typical supplement doses, but higher doses may cause digestive upset like diarrhea, loose stools, and gastrointestinal discomfort. Safety during pregnancy is not well established enough, so resveratrol supplements should be avoided in pregnancy; there's also not enough data to say it's safe while breastfeeding.
CoQ10 is generally well tolerated by most adults; minor side effects like digestive upset (appetite loss, diarrhea, nausea, vomiting), headache, and dizziness are rare and typically affect less than 1% of users. Pregnancy safety is unclear, so CoQ10 should be avoided unless your doctor advises otherwise; breastfeeding safety hasn't been well studied, so check with your doctor if you're nursing.

Before taking Total Mitochondria, check with your pharmacist if you're on warfarin or other blood thinners (anticoagulants) or antiplatelet drugs — CoQ10 could reduce warfarin's effectiveness and resveratrol may increase bleeding risk. Also flag any medications broken down by your liver (CYP2C19, CYP1A2, CYP3A4, or CYP2E1 substrates), blood pressure medications, or chemotherapy drugs.
Most of these interactions are theoretical, but resveratrol did increase one drug's (chlorzoxazone) levels by about 54% in a study.

Resveratrol's biggest concern is its theoretical effect on several liver enzymes (CYP2C19, CYP1A2, CYP3A4, and CYP2E1) that break down many drugs — meaning it could raise levels of medications metabolized by these pathways. The evidence here is mostly from lab research; human cases haven't been widely reported except for one pharmacokinetic study showing a 54% increase in chlorzoxazone levels.
Additionally, resveratrol may have antiplatelet effects and could increase bleeding risk if you're on blood thinners (anticoagulants) or antiplatelet drugs like aspirin.
CoQ10 carries a Moderate interaction risk with warfarin (a blood thinner) — there are four reported cases where CoQ10 appeared to reduce warfarin's effectiveness, likely because CoQ10 is chemically similar to vitamin K and may promote clotting. It also theoretically could reduce the effectiveness of certain chemotherapy drugs that depend on generating free radicals, and may have additive blood-pressure-lowering effects with blood pressure medications (a Minor concern).

MicroActive Resveratrol
Anticoagulant/Antiplatelet Drugs
Resveratrol may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Resveratrol seems to have antiplatelet effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that resveratrol can inhibit CYP1A2 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Resveratrol might increase levels of drugs metabolized by CYP2E1.
In vitro research suggests that resveratrol inhibits CYP2E1 isoenzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.
MicroActive CoQ10
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
