Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Cenobamate already tends to make people feel drowsy, dizzy, or just slowed down, especially when you're first starting it or moving up in dose. 5-HTP can add its own sleepy effect on top of that, since it raises serotonin and has been linked to drowsiness in studies. Stack the two and you may feel more tired, foggy, or unsteady on your feet than you would on Cenobamate alone.

I'll be honest, this concern is mostly theoretical, based on how each one behaves rather than strong head-to-head studies. Cenobamate isn't a prodrug, so 5-HTP isn't changing how it's activated, it's just a possible piling-on of sedation. Don't change anything on your own. A quick chat with your pharmacist or doctor is the safest way to sort this out.

From our pharmacist’s overview on the Cenobamate page

“Anyone taking cenobamate (Xcopri) for seizures already knows the drug can cause drowsiness on its own, which is exactly why this list warrants genuine vigilance before adding a supplement. Of the 122 ingredients with an interaction on file, seven rate major, and the pattern is lopsided: 78 are sedating in their own right, from kava dow…”

5-HTP is a compound your body uses to make serotonin, and people take it as a supplement hoping to improve mood, sleep, and headaches.
