This combination pill pairs hydralazine (which relaxes your blood vessels) with reserpine (which calms certain nerve signals), and both work to bring blood pressure down. The concern here is simple: andrographis may have a mild blood-pressure-lowering effect of its own. Stack that on top of your medication and your pressure could dip a bit lower than expected.

If that happens, you might feel dizzy, lightheaded, or tired, especially when standing up quickly. I want to be honest with you, though: this is a theoretical concern based only on animal research, not human studies, so it may not amount to much. Still, since you're on a real blood pressure medicine, check in with your pharmacist or doctor before adding andrographis, and let them know if you notice any of those symptoms.
