Fluvoxamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.

Here's the gist for these two together. Fluvoxamine (Luvox) is processed by a liver enzyme called CYP1A2, and lab studies hint that ashwagandha may speed up that enzyme. In theory, a faster enzyme could clear fluvoxamine a little quicker, which might leave slightly less of it working in your body. If that happened, you could notice your OCD or anxiety symptoms creeping back, or feel the medicine isn't holding you as steadily.

The important part: this is based only on test-tube research, not real people, and it's rated a minor, possible interaction. So I wouldn't lose sleep over it. Just don't start or stop anything on your own. Mention the ashwagandha to your pharmacist or prescriber so they can keep an eye on how you're feeling.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the gist for fluvoxamine (Luvox): it's an SSRI that raises serotonin in your brain to ease OCD and social anxiety. Ashwagandha has been shown in animal studies to nudge serotonin pathways too, so the theory is that stacking the two could add up and push serotonin a bit higher than intended.

I want to be honest about how thin this evidence is. There are no human case reports of problems, and the rating on file is minor. Still, since both touch serotonin, it's worth watching for signs like jitteriness, agitation, shaking, sweating, a racing heart, or feeling unusually restless. If anything like that pops up after adding ashwagandha, loop in your pharmacist or doctor before making any changes.

What Is Fluvoxamine Used For?

Brand name: Luvox

Both fluvoxamine maleate tablets and Fluvoxamine Maleate extended-release capsules are approved to treat obsessive-compulsive disorder (OCD) — a condition marked by unwanted, recurring thoughts (obsessions) and/or repetitive behaviors (compulsions) that cause significant distress or interfere with daily life.

Fluvoxamine maleate tablets are approved for adults and for children and adolescents ages 8 to 17. Fluvoxamine Maleate extended-release capsules are approved for adults with OCD; their use in pediatric patients naive to fluvoxamine requires special prescriber consideration regarding the lowest available dose.
