Ashwagandha and Frovatriptan

Here is the interaction we found between these two, with the evidence behind it.

Minor

Serotonergic Drugs

Why this applies to Frovatriptan

Frovatriptan falls under Serotonergic Drugs — the group Ashwagandha interacts with.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Frova (frovatriptan) and ashwagandha. Frovatriptan works on serotonin receptors to ease your migraine. Some animal research hints that ashwagandha may also nudge serotonin pathways, so in theory stacking the two could add up. That raises a small, mostly theoretical concern about too much serotonin activity, which can show up as agitation, a racing heart, sweating, shakiness, or feeling flushed.

The honest truth is the evidence here is weak. There are no reported cases of serotonin syndrome from this combo, and the interaction is rated minor. I wouldn't lose sleep over it, but since both touch serotonin, it's worth a quick chat with your pharmacist or doctor before adding ashwagandha, especially if you take other serotonin-affecting medicines.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Frovatriptan page

“If you take frovatriptan (Frova) for migraine attacks, lithium is the interaction to understand first. Frovatriptan itself works through serotonin, and lithium, one of two major-rated entries among the 38 supplements on file, can both mask and raise the risk of serotonin syndrome.”
