Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for ondansetron (Zofran) and ashwagandha. Lab studies hint that ashwagandha may speed up a liver enzyme called CYP1A2, which is one of the pathways your body uses to clear ondansetron. If that enzyme runs faster, the medicine could theoretically be cleared a bit quicker, meaning it might not control your nausea and vomiting as well. Ondansetron is not a prodrug, so faster clearance means less effect, not more.
I want to be honest about how thin this evidence is. It comes from test-tube and animal work, not real people, and we've rated it a minor concern. Most folks won't notice anything. Still, if you're relying on Zofran during chemo or after surgery, mention the ashwagandha to your pharmacist or doctor.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two. Ondansetron (Zofran) is partly handled by a liver enzyme called CYP3A4. Lab studies hint that ashwagandha may speed up that enzyme, which could clear ondansetron a little faster. In theory, that might make your Zofran slightly less effective, meaning your nausea could break through more easily.
I want to be honest about how soft this evidence is. This concern comes only from test-tube research, not from real people, and the interaction is rated minor. Ondansetron isn't a prodrug, so it doesn't flip in the other direction. For most folks this won't be a noticeable problem, but if your nausea isn't well controlled while using both, give your pharmacist or doctor a quick call.

Serotonergic Drugs

Here's the short version: ondansetron (Zofran) works on serotonin in your body to calm nausea. Ashwagandha may also nudge serotonin pathways, at least in animal studies. So in theory, pairing them could add a little serotonin activity on top of each other.
I want to be honest with you though, this is a very mild, theoretical concern. There are no real-world case reports of problems when ashwagandha is combined with serotonin drugs like this one, and ondansetron rarely causes serotonin issues on its own. If you ever noticed things like agitation, fast heartbeat, sweating, shivering, or muscle twitching, that's worth a call. Otherwise, just let your pharmacist or doctor know you take both so they can keep an eye on it.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. What Is Ondansetron Used For?
Brand name: Zofran
Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.
Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
