Why this applies to Clomipramine

Clomipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the situation with Anafranil (clomipramine) and ayahuasca. Your body relies partly on an enzyme called CYP2D6 to clear clomipramine. The beta-carboline compounds in ayahuasca (harmine and harmaline) may slow that enzyme down, so in theory clomipramine could build up higher than usual. That could mean more of its typical side effects, like a dry mouth, constipation, drowsiness, dizziness, a racing heartbeat, or problems with heart rhythm.

I'll be honest though: this CYP2D6 concern is based only on lab studies and hasn't been shown in real people. The bigger worry is separate. Ayahuasca acts like an MAOI, and combining MAOIs with a serotonin drug like clomipramine can trigger dangerous serotonin toxicity. Please don't mix these without talking to your doctor or pharmacist first.

Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the thing to know about pairing ayahuasca with clomipramine. The beta-carboline compounds in the brew may block an enzyme called CYP3A4, which is one of the pathways your body uses to clear clomipramine. If that pathway slows down, more of the drug can hang around in your blood, which could mean stronger side effects like a dry mouth, constipation, drowsiness, blurry vision, a racing heartbeat, or feeling lightheaded when you stand up.

I want to be honest though: this particular concern comes only from lab studies, not from real people, so it's theoretical right now. The bigger worry is something different. Clomipramine raises serotonin, and ayahuasca acts like an MAOI, so combining them risks a dangerous serotonin reaction. Please don't mix these without talking to your doctor or pharmacist first.
