Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP3A4 substrates.
Ayahuasca contains beta-carboline alkaloids. In vitro research suggests that beta-carboline alkaloids competitively inhibit CYP3A4 enzymes. This effect has not been reported in humans.

Here's the gist for Tibsovo (ivosidenib): your body relies on a liver enzyme called CYP3A4 to clear this drug out. Ayahuasca contains beta-carboline compounds that, in lab studies, can slow that same enzyme down. If that happened in a person, ivosidenib could build up in the blood and hit harder than intended.

Why that matters with this drug: ivosidenib can affect your heart rhythm (a QT-prolonging effect), so higher levels could raise the risk of dangerous rhythm changes, plus more nausea, fatigue, or other side effects. I'll be honest though, this concern is theoretical and only seen in test-tube research, not in real patients. Even so, with a cancer medicine like this, please don't combine them without first checking with your oncologist or pharmacist.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
