Why this applies to Perphenazine

Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ayahuasca interacts with.

Theoretically, ayahuasca might increase plasma concentrations of CYP2D6 substrates.
Ayahuasca contains beta-carboline alkaloids, including harmine and harmaline. In vitro research suggests that the beta-carboline alkaloids might inhibit CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist for perphenazine (Trilafon). Your body relies on a liver enzyme called CYP2D6 to help clear this drug out. The beta-carboline compounds in ayahuasca (harmine and harmaline) can block that enzyme, at least in lab studies. If that same blocking happens in people, perphenazine could build up and hit harder than expected.

What might that look like? More of perphenazine's usual effects, like heavy drowsiness, dizziness, low blood pressure, and movement problems such as stiffness, restlessness, or muscle spasms. I want to be honest, though: this concern comes from test-tube data only, not from real patients, so it's theoretical. On top of that, ayahuasca itself can be risky and is often illegal. Please loop in your doctor or pharmacist before mixing anything.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
