Aspirin, Caffeine, Dover's Pwd falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Baikal Skullcap interacts with.
In vitro evidence suggests that constituents of Baikal skullcap inhibit the activity of CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product mixes aspirin, caffeine, and Dover's powder (which contains ipecac and an opioid). The piece that matters here is the caffeine, which your liver clears using an enzyme called CYP1A2. In lab studies, compounds in Baikal skullcap appear to slow that same enzyme down. If that happened in your body, caffeine could hang around longer and build up a bit.
In practice, that might mean more of caffeine's usual effects: feeling jittery, a racing or pounding heartbeat, trouble sleeping, or an upset stomach. That said, this is only a theoretical concern seen in test tubes, not in real people, so the actual risk is likely small. Still, it's worth a quick chat with your pharmacist or doctor before pairing the two.

Preliminary clinical research suggests that taking capsules containing a combination of astragalus, goldthread, and Baikal skullcap daily for 4 weeks inhibits platelet aggregation; the effect seems to be similar to that of aspirin 50 mg daily. It is unclear if this effect is due to Baikal skullcap, other ingredients, or the combination.

The combo product you're asking about contains aspirin, which already thins the blood a bit by keeping platelets from clumping together. Baikal skullcap may do something similar. In early lab work (and one small human study using a multi-herb blend), the herb's flavonoids seemed to slow platelet activity too. Stack that on top of aspirin and you could, in theory, nudge your bleeding risk a little higher.
To be clear, this is mostly theoretical and based on weak evidence, so I don't want you to panic. But it's worth watching for things like easy bruising, bleeding gums, nosebleeds, or dark stools. Before adding the supplement, just check with your pharmacist or doctor so they can weigh it for you.
