Theoretically, Baikal skullcap might increase levels of drugs metabolized by CYP2C19 enzymes.
In vitro evidence suggest that wogonin, a constituent of Baikal skullcap, modestly inhibits the activity of CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist with Clomipramine (Anafranil) and Baikal skullcap. Your liver uses an enzyme called CYP2C19 to help break clomipramine down. One compound in skullcap, called wogonin, may gently slow that enzyme. In theory, that could let clomipramine build up a bit more in your body. Clomipramine is an active drug (not a prodrug), so higher levels mean stronger effects, things like drowsiness, dry mouth, constipation, blurry vision, dizziness, or a faster heartbeat.

The big caveat: this is based only on lab studies, not real people, so the actual risk is uncertain and likely small. Still, since clomipramine needs careful dosing, it's worth a quick chat with your pharmacist or doctor before adding skullcap.

Theoretically, Baikal skullcap may increase levels of drugs metabolized by CYP1A2 enzymes.

Here's the basic idea: your body uses a liver enzyme called CYP1A2 to help break down clomipramine. In lab studies, compounds in Baikal skullcap (like baicalin and baicalein) seem to slow that enzyme down. If that happened in real people, clomipramine could clear more slowly and build up in your system, which may mean stronger effects and more side effects, things like dry mouth, constipation, drowsiness, dizziness, a racing heartbeat, or trouble urinating.

The honest part: this is only seen in test-tube research, not in actual patients, so it's theoretical right now. Clomipramine is a drug where blood levels matter, so I'd still keep your pharmacist or doctor in the loop before adding this herb rather than changing anything on your own.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
