Treprostinil (Inh) falls under ANTIHYPERTENSIVE DRUGS — the group Baikal Skullcap interacts with.
Animal research suggests that baicalein, a constituent of Baikal skullcap, might lower blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Inhaled treprostinil (Tyvaso) works partly by relaxing and widening blood vessels, so it can already nudge your blood pressure down. Baikal skullcap, in animal studies, seems to do a little of the same thing. Put them together and the concern is that their effects could stack, making your pressure drop more than expected.
If that happened, you might feel lightheaded, dizzy, faint when standing up, flushed, or unusually tired. I want to be honest though: this is a theoretical worry based on lab and animal work, not solid human proof. Still, since treprostinil already affects blood pressure, it's worth a quick chat with your pharmacist or doctor before adding this herb rather than starting it on your own.

Preliminary clinical research suggests that taking capsules containing a combination of astragalus, goldthread, and Baikal skullcap daily for 4 weeks inhibits platelet aggregation; the effect seems to be similar to that of aspirin 50 mg daily. It is unclear if this effect is due to Baikal skullcap, other ingredients, or the combination.

Here's the thing to keep in mind with inhaled treprostinil (Tyvaso): besides opening up the blood vessels in your lungs, it also makes your platelets a little less sticky, so it gently lowers your body's ability to form clots. Baikal skullcap may do something similar. In lab studies, its compounds (like baicalin) seem to slow platelet clumping, and one small human study using a skullcap-containing blend showed an aspirin-like effect.
Stacked together, the worry is a slightly higher chance of bleeding or bruising, think nosebleeds, bleeding gums, or bruises that show up easily. This is theoretical and based on weak evidence, so don't panic, but please run it by your pharmacist or doctor before adding the herb.

What Is Treprostinil Oral Inhalation Used For?

Treprostinil oral inhalation is used to treat two serious lung conditions that raise pressure in the blood vessels of the lungs. It helps improve how much physical activity you can do.
Tyvaso inhalation solution and Tyvaso DPI are both indicated to treat pulmonary arterial hypertension (PAH; WHO Group 1) and pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3). Clinical studies establishing effectiveness in PAH primarily included patients with moderate symptoms (NYHA Class III) whose PAH was idiopathic, heritable, or related to connective tissue disease. Studies in PH-ILD primarily included patients with idiopathic interstitial pneumonia, combined pulmonary fibrosis and emphysema, or connective tissue disease-related lung disease.
