Oral treprostinil (Remodulin) widens your blood vessels to ease the workload on your heart and lungs, and one of its built-in effects is lowering blood pressure. Here's the concern with Baikal skullcap: in animal studies, one of its compounds (baicalein) seemed to nudge blood pressure down too. Put the two together and they could, in theory, stack up.

If your pressure drops too far, you might feel dizzy, lightheaded, flushed, or get a headache, and standing up quickly could make it worse. I want to be honest though, this is based on lab and animal work, not solid human studies, so it's a possibility and not a sure thing. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding this herb.

Theoretically, Baikal skullcap might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Preliminary clinical research suggests that taking capsules containing a combination of astragalus, goldthread, and Baikal skullcap daily for 4 weeks inhibits platelet aggregation; the effect seems to be similar to that of aspirin 50 mg daily. It is unclear if this effect is due to Baikal skullcap, other ingredients, or the combination.

Here's the thing to keep in mind with oral treprostinil (Remodulin) and Baikal skullcap. Treprostinil itself isn't a blood thinner, but it does relax blood vessels and can slow how your platelets clump together. That already nudges you toward easier bruising and bleeding. Baikal skullcap, at least in lab and early human studies, may also make platelets a little less sticky, so stacking the two could in theory add up and raise your bleeding risk.

I'll be honest, the evidence here is weak and mostly from test-tube work or a mixed-herb study, so this is more a caution than a sure thing. Still, watch for unusual bruising, nosebleeds, or bleeding gums, and please check with your doctor or pharmacist before adding this herb.

About Treprostinil

What Is Treprostinil Used For?

Brand names: Remodulin, Orenitram

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
