Aspirin, Dipyridamole falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Black Tea interacts with.
Black tea contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, the caffeine in black tea might increase the risk of bleeding when used concomitantly with antiplatelet drugs. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Aggrenox already combines two blood thinners: aspirin and dipyridamole, both of which keep your platelets from clumping. The idea here is that the caffeine in black tea may have its own mild antiplatelet effect, so stacking it on top of an already platelet-blocking medicine could, in theory, nudge your bleeding risk a little higher. In real life that might look like easier bruising, nosebleeds, or bleeding gums.
I want to be honest with you though: this concern comes only from lab studies, and it has not actually been seen in people drinking tea. A normal cup or two is very unlikely to cause trouble. If you notice unusual bruising or bleeding, or you drink a lot of strong tea, check in with your pharmacist or doctor.

DIPYRIDAMOLE (Persantine)
2 of 2Black tea contains caffeine. Caffeine is a methylxanthine that may inhibit dipyridamole-induced vasodilation. It is recommended that methylxanthines such as caffeine, as well as methylxanthine-containing products such as black tea, be stopped 24 hours prior to pharmacological stress tests. Methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the situation with Aggrenox (which combines aspirin and dipyridamole) and your daily cup of black tea. The caffeine in tea is what we call a methylxanthine, and it can blunt the way dipyridamole widens your blood vessels. For everyday use as a blood thinner, your regular tea habit isn't a major concern, and there's no need to panic.
Where this really matters is if you're scheduled for a pharmacological stress test that uses dipyridamole. Caffeine can interfere with that test and make the results less reliable. That's why doctors usually ask you to avoid black tea, coffee, and other caffeine for about 24 hours beforehand. If you have a stress test coming up, check the exact instructions with your doctor or pharmacist.
