Why this applies to Acetaminophen, Butalbital, Caffeine

Acetaminophen, Butalbital, Caffeine falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

The piece of your headache combo I'd watch here is the acetaminophen (the Tylenol part). Your body clears acetaminophen partly through a process called glucuronidation. In a lab dish, a compound in breadfruit leaves (artocarpin) seemed to slow that pathway down. In theory, slower clearance could let acetaminophen build up a little more than usual, which over time is mainly a concern for the liver.

Here's my honest take though: this is test-tube evidence only, never actually shown in people, so I wouldn't lose sleep over it. Eating breadfruit as food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger unknown. If you're using a medicinal breadfruit product regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on your total acetaminophen.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Butalbital, Caffeine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the short version on combining breadfruit with your headache medicine (the acetaminophen, butalbital, and caffeine combo). One of breadfruit's compounds called artocarpin may slow down a liver enzyme known as CYP3A4, which helps process butalbital. In theory, slowing that enzyme could let a little more of the butalbital build up, which might mean extra drowsiness, dizziness, or feeling foggy.

The important thing to know is that this is very weak, theoretical evidence from lab dishes and animals, not people. Eating breadfruit as a food almost certainly won't matter. Concentrated leaf teas or extracts are the bigger question mark. If you plan to use those medicinally, just give your pharmacist or doctor a heads up first.

What Is Acetaminophen, Butalbital, and Caffeine Used For?

Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).

All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
