Why this applies to Brentuximab Vedotin

Brentuximab Vedotin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist with breadfruit and Adcetris (brentuximab vedotin). After this drug is infused, an enzyme called CYP3A4 helps break down the potent chemo payload (called MMAE) that it carries. In lab and animal studies, certain breadfruit leaf compounds can mildly slow that same enzyme. In theory, slowing it could let more of that toxic payload linger, which might bump up side effects like low blood counts, nerve tingling or numbness in the hands and feet, nausea, and fatigue.

That said, this is very weak, theoretical evidence, and we don't know if it matters in real people. Eating breadfruit as food is unlikely to be an issue. Concentrated leaf teas or extracts are the bigger question mark, so please run those by your oncology team or pharmacist first.

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
