Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist for cobimetinib (Cotellic). Your body relies on an enzyme called CYP3A4 to clear this medication out. Some lab and animal studies hint that a compound in breadfruit leaves (artocarpin) may gently slow that same enzyme down. If that happened in real life, cobimetinib could build up a bit, which in theory might mean more of its known side effects, like diarrhea, rash, fatigue, fever, or changes in vision.

The honest truth is that this is theoretical and only based on test-tube and animal data, so we don't know if it matters in people. Eating breadfruit as a food is very unlikely to be an issue. Concentrated leaf teas or extracts are the bigger question mark. Since cobimetinib is a cancer drug, please run any breadfruit supplement past your oncologist or pharmacist first.

About Cobimetinib

What Is Cobimetinib Used For?

Brand name: Cotellic

Cobimetinib is approved for two uses in adults. First, it is used in combination with another drug called vemurafenib to treat melanoma (a type of skin cancer) that cannot be surgically removed or has spread to other parts of the body — but only when the tumor carries a specific genetic change called a BRAF V600E or V600K mutation. A lab test must confirm this mutation before treatment begins.

Second, cobimetinib can be used on its own (as a single agent) to treat adults with histiocytic neoplasms — a rare group of cancers involving immune cells called histiocytes.
