Why this applies to Eliglustat

Eliglustat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear eliglustat (Cerdelga) from your system. Lab and animal studies hint that breadfruit, mainly a compound in it called artocarpin, might weakly slow that enzyme down. If that happened in people, less eliglustat would get broken down and levels could creep up, which matters because this drug already carries a known risk of affecting heart rhythm (a longer QT interval) at higher levels.

That said, this is theoretical and based only on test-tube and animal work, not real patients. Eating breadfruit as a food is very unlikely to cause trouble. The bigger question is concentrated leaf teas or extracts. Since eliglustat is sensitive to CYP3A4 changes, please run any breadfruit supplement past your pharmacist or doctor first.

About Eliglustat

What Is Eliglustat Used For?

Brand name: Cerdelga

Cerdelga (eliglustat) is used for the long-term treatment of Gaucher disease type 1 in adults. Gaucher disease type 1 is a genetic condition in which a fatty substance called glucosylceramide builds up in organs like the liver, spleen, and bone marrow, causing them to enlarge and leading to problems like anemia and low platelet counts.

Cerdelga is only appropriate for adults whose bodies process the drug in a predictable way — specifically those identified as CYP2D6 extensive metabolizers (EMs), intermediate metabolizers (IMs), or poor metabolizers (PMs) by an FDA-cleared genetic test. People who are CYP2D6 ultra-rapid metabolizers may not reach drug levels high enough to benefit, and Cerdelga is not recommended for them.
