Why this applies to Dofetilide

Dofetilide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the basic idea: lab and animal studies hint that compounds in breadfruit (mostly from the leaves) might mildly slow down a liver enzyme called CYP3A4. Since some of dofetilide is handled by that enzyme, the theory is that breadfruit could let a little more of the drug linger in your system. With dofetilide, that matters more than with most medicines, because it's a drug where even small shifts in level can affect your heart rhythm and, rarely, trigger a dangerous arrhythmia.

That said, I want to be honest with you: this is only a theoretical concern from test-tube and animal work, not from people, and eating breadfruit as a food is very different from taking concentrated leaf extracts. Dofetilide is closely monitored by your cardiologist anyway, so just mention any breadfruit supplements to your doctor or pharmacist before starting them.

About Dofetilide

What Is Dofetilide Used For?

Brand name: Tikosyn

Dofetilide capsules are used in two main ways for people with atrial fibrillation (AF) or atrial flutter (AFl) — abnormal, often rapid heart rhythms. First, they can help convert these abnormal rhythms back to a normal heart rhythm. Second, once a normal rhythm has been restored, dofetilide can help maintain it and delay the time before the abnormal rhythm comes back.

Because dofetilide can cause dangerous heart rhythm problems of its own, it is reserved for patients whose atrial fibrillation or atrial flutter causes significant symptoms. It has not been shown to be effective for paroxysmal atrial fibrillation (short, self-terminating episodes). The brand Tikosyn carries the same two approved uses.
