Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by cytochrome P450 2C8 (CYP2C8) enzymes. So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits CYP2C8 enzymes, which might increase the levels of CYP2C8 substrates. Some drugs metabolized by CYP2C8 include amiodarone (Cardarone), paclitaxel (Taxol); nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Voltaren) and ibuprofen (Motrin), rosiglitazone (Avandia), and others.

Here's the gist with Vosevi (sofosbuvir, velpatasvir, voxilaprevir). A compound found in breadfruit leaves, called artocarpin, was shown in lab dish studies to slow down an enzyme called CYP2C8. In theory, slowing that enzyme could let certain drugs build up to higher levels in your body. The concern is that higher levels of voxilaprevir in particular might mean more side effects, such as headache, nausea, diarrhea, or tiredness.

I want to be honest about how shaky this is. It has only been seen in test tubes, never reported in real people, and eating breadfruit as a food is a different thing than taking concentrated leaf extracts or teas. Because you're treating hepatitis C and want this regimen to work right, just give your pharmacist or doctor a heads-up before using any breadfruit supplements.

Sofosbuvir, Velpatasvir, Voxilaprevir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist with Vosevi (sofosbuvir, velpatasvir, voxilaprevir) and breadfruit. A compound in breadfruit may slow down a liver enzyme called CYP3A4, and parts of Vosevi are handled by that enzyme. In theory, slowing it down could let a bit more of the medicine build up, which might nudge up side effects like headache, tiredness, nausea, or diarrhea.

The important part: this concern comes only from lab and animal studies, not from people, so it's rated as a minor, uncertain interaction. Eating breadfruit as a normal food is very unlikely to cause trouble. Concentrated leaf teas or extracts are the bigger question mark. Since finishing your hepatitis C course matters, just run any breadfruit supplements past your pharmacist or doctor first.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.

Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
