Tamoxifen Citrate falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

Here's the gist with breadfruit and tamoxifen. Your body partly clears tamoxifen and its active metabolites by attaching a sugar molecule to them, a process called glucuronidation (handled by UGT enzymes). A compound found in breadfruit, artocarpin, was shown in lab dish studies only to slow those UGT enzymes. In theory, that could let tamoxifen build up a bit, which might mean more of its usual effects like hot flashes, leg cramps, or nausea.

I want to be honest, though: this has never actually been seen in people, and eating breadfruit as a normal food is a different story than taking concentrated leaf teas or extracts. Since tamoxifen is so important for your treatment, please run any breadfruit supplements past your oncologist or pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Tamoxifen Citrate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's something worth knowing about tamoxifen: it's actually a prodrug, meaning your body has to convert it into its active forms (like endoxifen) before it can do its job fighting breast cancer. CYP3A4 is one of the enzymes involved in that activation, alongside CYP2D6. The concern on file is that breadfruit might block CYP3A4 a little.

If that blocking happened in a meaningful way, it could theoretically lower how much active medicine you get, which matters for a drug you rely on to lower cancer risk. That said, this is purely theoretical, based only on lab and animal studies, and rated minor. Eating breadfruit as food is very unlikely to matter. Concentrated leaf teas or extracts are the bigger question mark, so please run those by your oncologist or pharmacist first.

About Tamoxifen

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
