These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Caffeine interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 2 of those categories, so it can interact with Caffeine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
ANTICOAGULANT/ANTIPLATELET DRUGS
1 of 2Why this applies to Aspirin, Caffeine, Codeine Phosphate
Aspirin, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Caffeine interacts with.
Caffeine is reported to have antiplatelet activity. Theoretically, it might increase the risk of bleeding when used concomitantly with these agents; however, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the interesting wrinkle with this particular product: it already contains caffeine right in the formula, alongside aspirin and codeine. The concern on file is that caffeine may have mild antiplatelet activity, meaning it could in theory make it a little harder for your blood to clot. Since aspirin also thins the blood by blocking platelets, stacking extra caffeine on top might, in theory, nudge up your bleeding risk. Think easy bruising, nosebleeds, or stomach upset that lingers.
I want to be honest about the strength of this, though. This comes only from lab and animal data, and it has never actually been reported in real people. A normal cup or two of coffee is very unlikely to matter. If you take this regularly or use high-dose caffeine pills, just mention it to your pharmacist.

STIMULANT DRUGS
2 of 2Aspirin, Caffeine, Codeine Phosphate also falls under STIMULANT DRUGS.
Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's something easy to miss: this combination pill (Anacin with Codeine) already contains caffeine right alongside the aspirin and codeine. So if you're also drinking coffee, energy drinks, or taking a separate caffeine supplement, you're essentially stacking caffeine on top of caffeine.
That extra stimulant load is what this interaction is about. Too much caffeine together can leave you feeling jittery, anxious, or restless, with a racing or pounding heart and trouble sleeping. This is more of a theoretical, common-sense caution than a dangerous reaction, and a normal cup of coffee is unlikely to be a big deal for most people. Still, if you notice your heart racing or you feel wired, ease back on outside caffeine and check with your pharmacist or doctor.

“Anacin with Codeine combines aspirin, caffeine, and codeine, and each of the three picks up its own supplement problems, so this is a list worth real caution. Just over half of the 398 ingredients here, 202 of them, can add to aspirin's blood-thinning action and raise bleeding risk.”
