Caffeine and Cyproterone Acetate, Ethinyl Estradiol

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Caffeine interacts with whole categories of medication. Cyproterone Acetate, Ethinyl Estradiol falls into 2 of those categories, so it can interact with Caffeine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Why this applies to Cyproterone Acetate, Ethinyl Estradiol

Cyproterone Acetate, Ethinyl Estradiol falls under ESTROGENS — the group Caffeine interacts with.

Theoretically, estrogens might increase the levels and adverse effects of caffeine.
Estrogen inhibits caffeine metabolism.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know with Diane-35: the ethinyl estradiol part can slow down how your body clears caffeine. Estrogen gently blocks the liver enzyme (CYP1A2) that breaks caffeine down, so the caffeine you drink can stick around longer and hit a little harder than it normally would.

In real life, that means your usual coffee, tea, or energy drink might leave you feeling more wired, jittery, or restless, with a racing heart or trouble sleeping. It's not dangerous for most people, and it builds gradually rather than all at once. If you notice these symptoms, simply easing back on caffeine usually does the trick. There's no need to stop your pill, but feel free to check in with your pharmacist or doctor if it's bothering you.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 2

Cyproterone Acetate, Ethinyl Estradiol also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: the estrogen in your Diane-35 (ethinyl estradiol) can gently slow down a liver enzyme called CYP1A2, which happens to be the same enzyme that clears caffeine from your body. So when you drink coffee, tea, or energy drinks while on this pill, the caffeine may hang around a little longer than usual.

In plain terms, that means your usual cup might hit a bit harder. You may notice more jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious. The good news is this is rated minor and is mostly based on lab studies, not strong real-world data. Most people do fine. If caffeine feels stronger than normal, just cut back a bit and chat with your pharmacist.
