Caffeine and Isoproterenol

Here is the interaction we found between these two, with the evidence behind it.

Moderate

BETA-ADRENERGIC AGONISTS

Why this applies to Isoproterenol

Isoproterenol falls under BETA-ADRENERGIC AGONISTS — the group Caffeine interacts with.

Theoretically, large amounts of caffeine might increase the cardiac inotropic effects of beta-agonists.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Isoproterenol (Isuprel) is a beta-agonist, meaning it revs up your heart by stimulating the same beta receptors that adrenaline does. It speeds up your heart rate and makes each beat squeeze harder. Caffeine is a stimulant too, so the concern here is simple overlap: both can push your heart in the same direction at the same time.

The worry on file is that large amounts of caffeine could add to isoproterenol's effects on the heart, possibly leading to a faster or pounding heartbeat, palpitations, jitteriness, or feeling shaky. Keep in mind this is a theoretical concern based on how the two work, not strong human studies. A cup of coffee likely isn't a big deal, but heavy caffeine could matter. Since isoproterenol is usually given in a monitored setting, ask your care team if you have questions.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

About Caffeine

What is Caffeine?

  • Mental alertness and reducing fatigue
  • Improving athletic performance
  • Headache and migraine relief
  • Boosting focus and concentration
  • Weight loss aid

Caffeine is a natural stimulant found in coffee, tea, and many other plants and products. In moderate amounts it can boost alertness and reduce tiredness for most healthy adults, but too much can cause jitteriness, a racing heart, and sleep problems. Pregnant women and people with heart conditions or anxiety should be especially careful and check with a doctor or pharmacist.
